Skills: Knowledge of State and Federal regulations, including Assisted Living regulations and proficient in assessment and organizational skills.

Experience: A minimum of one-year experience in a related position normally required

SpiriTrust Lutheran® is now hiring a Resident Care Coordinator (LPN.)

Available Shifts:

Full Time Evening – 75 hours per bi-weekly pay

Part Time Evening – 30 hours per bi-weekly pay

Full Time Night – 75 hours per bi-weekly pay

Part Time Night – 30 hours per bi-weekly pay

General Responsibilities:

  1. Oversees internal and external health related services and administers appropriate health care of residents in accordance with the Assessment and Support Plan.
  2. Facilitates communication and documentation related to resident health care.
  3. Oversees and manages Resident Care Partners, supporting the development and maintenance of a strong health services program.
  4. Provides guidance, support and encouragement to the care team with a focus on vision and purpose
  5. Facilitates communication among the care team, supporting information sharing and problem-solving
